JURAT

A certification that you, the signer, swear/affirm that the contents of a document are true. The fee includes an oath/affirmation and a certificate.

PROOF OF EXECUTION

Proofs are used in place of an acknowledgement if the principal is unable to appear personally. A subscribing witness, who has also signed the document as a witness, must appear, on behalf of the principal and having taken the principal's acknowledgment that they signed a document, with ID and a credible identifying witness also known to the notary in order to complete this act. PHOTOCOPY OF A JOURNAL ENTRY

Upon a written request of a member of the public containing the name of the signer, the type of notarization, and the month and year of the notarization, a photocopy of a line item (or a notice that no such entry exists) will be provided at the indicated price.
